    Position Overview

    Lighthouse Homes Staff provide person-centered support to individuals with developmental disabilities, helping them live safely and independently in their homes and communities through Home and Community-Based Services (HCBS).

    • Assist individuals with activities of daily living (ADLs), self-care, mobility, and routine daily needs.

    • Promote independence, skill development, and community integration.

    • Support health, safety, and overall well-being in home and community settings.

    • Encourage participation in social, recreational, educational, and employment activities.

    • Follow individualized service plans developed through case management and family collaboration.

    • Maintain accurate documentation and clear communication with the support team.

    • Uphold confidentiality, ethical standards, and all state and federal HCBS regulations.

Lighthouse Transport Driver

  • (Non-Emergency Medical Transportation – NEMT)

    Position Overview
    Lighthouse Transportation Drivers provide safe, reliable, and courteous non-emergency medical transportation (NEMT) for individuals who need assistance traveling to medical appointments and essential destinations.

    Key Responsibilities

    • Transport clients safely to and from medical appointments, therapy sessions, pharmacies, and other approved locations.

    • Assist clients entering and exiting vehicles as needed, including mobility support.

    • Follow all state, company, and safety regulations related to NEMT services.

    • Maintain cleanliness, safety, and readiness of assigned vehicles.

    • Maintain accurate trip documentation and schedules.

    • Provide professional, respectful, and compassionate service to all passengers.

    • Communicate delays, concerns, or client needs to dispatch or supervisors promptly.

    • Ensure passenger comfort and safety throughout transport.

  • (Attendant, Personal, & Homemaking Services — Non-Medical Home Care)

    Position Overview
    Lighthouse Home Care Staff provide non-medical, in-home support to help clients maintain independence, comfort, and safety in their own homes. This role combines attendant care, personal care, and homemaking services based on each client’s individualized care plan.

    Key Responsibilities

    • Assist with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, ambulation, transferring, and positioning.

    • Provide personal care support such as hair care, nail care, skin care, and medication reminders (non-medical).

    • Perform homemaking tasks including meal planning and preparation, light housekeeping, laundry, bed making, dusting, vacuuming, and cleaning kitchens and bathrooms.

    • Assist with errands and essential shopping, including handling client funds when authorized.

    • Follow written care plans and any special diet plans as directed by healthcare providers.

    • Provide companionship and emotional support.

    • Ensure the client’s safety and security by supervising the home environment at all times.

    • Communicate effectively with supervisors and report any changes in client condition.
