I authorize Lighthouse Services LLC to verify the information contained in this application, including employment history and references, as permitted by law, and to conduct employment-related screenings required by state and federal regulations.

I understand that employment, if offered, may be contingent upon successful completion of required screenings and assessments, which may include criminal background checks, competency testing, tuberculosis screening, and verification of certifications, as applicable to the position.

If applying for or considered for a position involving transportation, I authorize review of my Motor Vehicle Record (MVR) and understand that a valid driver’s license and applicable insurance may be required.

I understand that proof of identity and authorization to work in the United States will be required in accordance with federal law if I am offered employment.

I acknowledge that I have reviewed, or have access to, the Privacy Policy and Terms and Conditions and agree to their provisions.

I agree to a physical examination, if requested, and understand that failure to meet any medical and/or health requirements for the position may prevent my employment with the Agency.  I also understand that employment, for certain positions, may be conditional upon successful completion of a Competency Test, negative Tuberculosis Test and a Criminal Background Check.

I further understand that, if hired, I may be required to provide proof that I am a citizen of the United States or proof that I am currently authorized to work in the United States.
